Prescribed forms
Prescribed forms 2 to 3 and 6 to 8, as set out in Schedule 7 of the Co-operatives Regulation 2005, are set out below for downloading.
Please note: the Form 1 Annual report required to be lodged by co-operatives is not available for downloading and must be obtained from the Registry.

| Form 2: Compulsory acquisition notice (Size: 8k) | Acquisition of shares pursuant to a notice to a dissenting shareholder that the transferee under a scheme or contract, for the purposes of section 360(1) of the Co-operatives Act 1992, wishes to acquire shares held by that shareholder. |
| Form 3: Notice to remaining shareholder (Size: 9k) | Notice to remaining shareholders by a transferee pursuant to a scheme or contract for the purposes of section 362(1)(a) of the Act. |
| Form 6: Report as to affairs (Size: 34k) | For the purposes of Schedule 4 to the Act (receivers, and other controllers, of property of co-operatives) - clauses 6A (1), 14(2)(b) and 15(1). |
| Form 7: Notification that a person has been appointed controller/ entered into possession etc (Size: 14k) | For the purposes of Schedule 4 to the Act (receivers, and other controllers, of property of co-operatives) - clauses 12(1)(a), (2)(a) and 3(a). |
| Form 8: Notification of appointment or cessation as an external administrator (Size: 17k) | For the purposes of Schedule 4 to the Act (receivers, and other controllers, of property of co-operatives) - clauses 12(4) and (6)(a). |
