Fair Trading Information Access Form - privacy collection notice

The Department of Customer Service, on behalf of the Commissioner for NSW Fair Trading, gives priority to protecting the privacy of your personal and/or health information. We do this by handling personal and/or health information in a responsible manner and in accordance with the Privacy and Personal Information Protection Act 1998 (PPIP Act) and the Health Records and Information Privacy Act 2002 (HRIP Act).

Your personal and/or health information is being collected by NSW Fair Trading for the following purposes:

  1. To process your application for information pursuant to Part 4, Division 1 of the Government Information (Public Access) Act 2009.
  2. Internal administrative purposes, including liaising with you in relation to your application.

Your personal and/or health information is provided voluntarily, however NSW Fair Trading may not be able to process your application if you do not provide it. We will not disclose your personal and/or health information without your consent, or unless we are otherwise authorised or required to by law. Our Privacy Statement describes when this may occur.
